Job summary

Baptist Memorial Health Care in Mississippi is seeking a Case Manager to apply systems, science, incentives, and information to improve healthcare practice and assist patients and their support systems to engage in a collaborative process designed to manage medical, social, and mental health conditions more effectively.

The case manager's objective is to achieve an optimal level of wellness for patients and improve coordination of care while providing cost effective, non-duplicative services.

Qualifications

• Graduate of School of Nursing (Accredited) required.
• Preferred: Bachelor’s in Nursing or Allied Health.
• RN licensure required in MS.

Responsibilities

• Assess and document patients' clinical, psychosocial and financial needs and plan care.
• Apply InterQual to determine Level of Service within 24 hours and coordinate with physicians.
• Identify plan of care, LOS and transition/discharge with patients and families.
• Coordinate with nursing and social work to ensure safe discharge planning.
• Monitor treatment plan and ensure timely discharge with no care gaps.
• Escalate barriers to appropriate treatment or transition.
• Maintain knowledge of facilities and resources for patients and caregivers.
