Responsibilities

• Assess and document the clinical, psychosocial and financial needs of patients and coordinate the plan of care.
• Apply InterQual criteria to determine Level of Service and ensure appropriate care within the first 24 hours.
• Identify the plan of care, length of stay, and discharge plan with patients and families; ensure safe discharge/transition.
• Coordinate with nursing, social work, and physicians to monitor the treatment plan and ensure timely transitions.
• Escalate barriers to appropriate treatment or discharge when needed to prevent delays.
