Qualifications

• BSN required or in lieu of BSN degree, a combination of education and clinical experience would be acceptable.
• Three years of relevant experience required.
• Active KY RN License required.

• Coordinate and complete discharge planning and care transitions.
• Ensure CMS and external review requirements are met and resources are optimized.
