Job summary

HCA Florida University Hospital is seeking a Registered Nurse Case Manager (PRN) to support patients and families through the care journey. You will coordinate services, develop care plans, and partner with physicians and nurses to ensure safe transitions and high-quality care.

Responsibilities include guiding patients through program orientation, coordinating education, and documenting assessments. Excellent collaboration with the Rehab Program Director and care team is essential to deliver

Qualifications

• Florida RN license is required.
• 1 year of RN experience is required.
• BLS (Basic Life Support) must be obtained within 30 days of start date.

Responsibilities

• Guide patients and families through orientation, discharge criteria, and rights.
• Coordinate education for patients, families, and caregivers to plan treatment.
• Develop and implement individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and communicate findings to the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote HCA Healthcare values in every interaction.
