Job summary

HCA Healthcare Trident Hospital in Summerville, SC seeks a Registered Nurse Case Manager to support patients and families through their care journey. You will coordinate services, resources, and individualized care plans with physicians, nurses, and leaders to ensure safe transitions and high-quality care.

Required ADN or higher; RN license (state or compact); 1–3 years hospital nursing experience; CM/Utilization Review certification preferred; InterQual knowledge preferred.

Qualifications

• CM experience or familiarity with case management processes.
• InterQual knowledge and utilization review familiarity is preferred.
• Strong communication and teamwork abilities for collaboration with care team.

Responsibilities

• Guide patients and families through orientation, explain rehabilitation and discharge criteria.
• Coordinate education for patients, families, and caregivers to support participation in treatment planning.
• Develop and implement individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and discharge planning evaluations and share findings with the team.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care.
• Promote organizational values in every patient interaction.
