Job summary

HCA Healthcare Summerville Hospital is seeking an RN Case Manager (PRN) to support patients and families through care transitions. You will coordinate services, develop care plans, and partner with physicians and the care team to ensure safe and effective care.

You will guide education for patients and families, document psychosocial assessments, and collaborate across disciplines while upholding HCA Healthcare values of compassion and excellence.

Qualifications

• Graduate from an accredited school of Nursing (ADN or higher)
• Current RN license in state of practice or active compact license
• 1–3 years of clinical/hospital nursing experience
• Prior experience in Case Management required for PRN role
• Case Management or Utilization Review certification preferred
• InterQual experience preferred

Responsibilities

• Guide patients and families through program orientation and discharge criteria
• Coordinate education for patients, families, and caregivers to participate in treatment planning
• Develop and implement individualized treatment plans reflecting patient needs
• Complete psychosocial assessments and discharge planning evaluations
• Collaborate with Rehab Program Director, Facility Case Management Director, and care team
• Promote HCA Healthcare’s values through patient and family interactions
