Qualifications

• The RN must be licensed in the practicing state.
• 1–3 years of direct clinical hospital nursing experience required.
• 2 years in Case Management preferred and InterQual experience preferred.

Responsibilities

• Coordinate the plan of care and manage length of stay with discharge planning.
• Identify patients at risk during transitions of care.
• Assess psychosocial, medical, and discharge needs with family involvement.
• Ensure post-acute medical needs match appropriate level of care.
• Refer to third-party payers, disease, and case management programs as needed.
• Act as liaison between physicians, patients, families, and hospital staff.
