Job summary

Grand Strand Medical Center in Myrtle Beach, SC, part of HCA Healthcare, is seeking a Registered Nurse (RN) Case Manager to join our team. The RN Case Manager coordinates care, facilitates discharge planning, and advocates for patients throughout the episode of care.

You will work with physicians, nurses, patients, and families to assess needs, manage resources, and ensure transitions of care align with InterQual criteria.

Qualifications

• Graduate from an accredited nursing school with ADN or higher; BSN preferred.
• Current RN license in the state of practice or an active Compact/Multi-State license.
• 1–3 years of clinical, hospital nursing experience required.
• 2 years of case management experience preferred; Case Management or Utilization Review certification preferred.
• InterQual experience preferred.

Responsibilities

• Coordinate the plan of care, manage length of stay, and develop discharge plans.
• Identify patients at risk during transitions of care between settings.
• Perform psychosocial, medical and discharge assessments for patients/families.
• Ensure post-acute care needs align with Social Services partnerships.
• Refer to third-party payer, disease, and case management programs as needed.
• Liaise with physicians, patients, families, hospital staff, and outside agencies to advocate for patients.
