Job summary

AccentCare is seeking an RN Case Manager in DeSoto County, MS, to oversee care plans from admission through discharge and coordinate with nurses and allied clinicians. This role combines direct and indirect patient care with leadership responsibilities.

The candidate should have an RN license in MS, at least 1 year of RN experience, and CPR certification, with a commitment to quality patient care and coordination across the care team.

Qualifications

• Current CPR Certification required.
• Graduate from an approved school of professional nursing and currently licensed to practice as an RN in the state of operation.
• Minimum of one year of experience as an RN.

Responsibilities

• Assess the status of nursing needs and manage the patient care plan.
• Complete initial patient assessment upon admission with history.
• Select appropriate nursing diagnoses.
• Review medications for potential adverse reactions and allergies.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions within the Nurse Practice Act.
• Re-evaluate patients' nursing and clinical needs as required.
• Document assessment, progress, and discharge notes in a timely manner.
• Inform physician and staff of changes in patient condition and needs.
