Job summary

TruHealth, a division of American Health Partners, seeks a Registered Nurse Case Manager to support care coordination for Medicare Advantage members. You will visit patients, collaborate with physicians, and ensure effective plans of care, with emphasis on geriatric services.

The role requires strong communication, self-motivation and teamwork; you’ll interact with multiple care teams and participate in audits to improve care delivery.

Qualifications

• Graduate from an accredited RN program.
• Bachelor’s degree preferred.
• Two years’ clinical nursing or geriatric rehabilitation experience.
• Two years in managed care; 3–5 years in case management within managed care.
• Current RN license; valid driver's license and auto insurance.

Responsibilities

• Manage assigned patient caseload.
• Complete health risk assessments for designated members.
• Initiate, reassess and revise plans of care with the health care team.
• Evaluate and plan patient care with the interdisciplinary team.
• Educate patients and families using available teaching resources.
• Coordinate outpatient discharge planning according to patient needs.
