Job summary

Sibley Memorial Hospital, part of Johns Hopkins Medicine in Washington, DC, seeks an experienced Case Manager to coordinate patient-centered care from pre-admission to post-discharge. You’ll collaborate with social workers, physicians, and the care team to create safe, efficient transitions and optimize outcomes in the Inpatient Med/Surg unit.

Requirements include a BSN and DC RN license with a minimum of two years in clinical nursing; CCM/ACM certification is preferred and CPR is required.

Qualifications

• BSN nursing program completed; RN licensed.
• Minimum 2 years clinical nursing; discharge planning preferred.
• ACM/CCM certification preferred.
• Licensed RN in DC.
• CPR required.

Responsibilities

• Coordinate patient-centered care from pre-admission to post-discharge.
• Identify and remove barriers to services with patient, family, and care team.
• Develop sustainable transition plans tailored to patient needs.
• Support Inpatient Med/Surg department.
