Qualifications

• Completion of an accredited BSN Nursing Program (RN) required.
• Minimum 2 years of experience in clinical nursing; minimum 1 year in case management or discharge planning preferred.
• ACM RN or CCM certification preferred.
• Licensed to practice as a Registered Nurse in the District of Columbia.
• CPR- Basic CPR required.

Responsibilities

• Coordinate patient-centered care across the continuum from pre-admission to post-discharge, leading and advocating in care transitions.
• Manage clinical resources and transition planning for patients within an assigned caseload, removing barriers to care.
