Job summary

PAM Health is seeking an RN Case Manager to coordinate care for a designated patient population and to review inpatient records for appropriate utilization of hospital resources. The role involves discharge planning and collaboration with an interdisciplinary team to drive quality outcomes.

Requires an active RN license in the state and current BLS certification. Inpatient experience (3–5 years) in acute, IRF, or LTACH settings is preferred, with a strong focus on payor relationships and care

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for a designated patient population by reviewing inpatient admission records and ensuring appropriate utilization of hospital resources.
• Assess discharge needs and coordinate the discharge plan.
• Work with the interdisciplinary team to achieve desired financial and quality outcomes.
• Maintain and enhance payor relationships.
