The position emphasizes experienced inpatient care within an IRF/LTACH setting, with preferred state licensure and current BLS certification.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include OT, PT, RN, RRT, SLP, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for a designated patient population with interdisciplinary teams.
• Review inpatient admission records to ensure proper utilization and level of care.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
