Qualifications

• RN license required.
• Three to five years of inpatient experience, preferably in acute, IRF, or LTACH setting.

Responsibilities

• Coordinate care across interdisciplinary teams for designated patients.
• Review inpatient admission records to ensure appropriate resource use.
• Assess discharge needs and coordinate the discharge plan.
• Collaborate with the interdisciplinary team to achieve health outcomes and financial targets.
• Maintain and strengthen payor relationships.
