Job summary

PAM Health Specialty Hospital of Stoughton in Massachusetts is seeking a dedicated Case Manager to coordinate care across disciplines for a defined patient population. You will review inpatient admission records to ensure appropriate resource use and determine level of care, while planning and coordinating discharge needs with the team.

Experience in inpatient settings (3–5 years) and active RN licensure with BLS certification are required.

Qualifications

• RN license required and active BLS certification.
• Three to five years inpatient experience preferred.
• Licensure in the state where hospital resides is preferred.

Responsibilities

• The Case Manager coordinates care across disciplines for a designated patient population.
• Reviews inpatient admission records to ensure proper utilization and determine level of care.
• Assesses discharge needs and coordinates discharge planning with the interdisciplinary team.
• Maintains and enhances payor relationships to support financial and quality outcomes.
