Job summary

Chapters Health System in Washington, DC area is seeking an experienced RN Case Manager to assess patient needs, coordinate care with the IDT, and deliver clinical and palliative support to keep patients at home.

The role requires an active RN license, at least one year nursing experience, and proficiency with EMR/EHR; candidates must have a valid driver\'s license and reliable transportation for home visits. Sign-on bonus and competitive salary are offered.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• PACE employees must have Alzheimer’s Disease and Related Dementias Training.
• Experience working with EMR/EHR systems.
• Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Ability to visit participant in their homes for assessments.
• Ability to perform essential functions including lifting with or without accommodation.
• Active BLS for healthcare professionals from AHA or Red Cross.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT.
• Coordinates the Plan of Care reflecting evolving needs.
• Educates patient, family, caregivers about disease, palliative care and safety.
• Home visits to assess home safety, medication, nutrition and DME compliance.
• Reports changes in patient condition to IDT or other health professionals.
• Participates with IDT to evaluate hospice referrals for level of care.
• Communicates with physicians and staff; supervises Home Health Aides as requested.
• During emergencies, may be required to report to work at designated location.
• Performs other duties as assigned.
