516 Visiting Nurse Association of Wisconsin, Inc. is seeking a Nursing Case Manager to coordinate home health services across the Milwaukee area. The role is full-time with a 40-hour week, and flexible scheduling including occasional weekend and on-call hours.

Ideal candidates are registered nurses with 3 years of clinical nursing experience, strong assessment and communication skills, and the ability to work with physicians, families, and the care team to implement effective plans of care.

Qualifications

• Registered Nurse license (state) with BLS certification.
• Typically requires 3 years of clinical nursing experience.
• Associate degree in nursing required; strong communication skills.

Responsibilities

• Assumes responsibility for caseloads in specialty areas.
• Conducts initial assessment to obtain physical, social and environmental data.
• Establishes the plan of care per physician orders and patient needs.
• Provides direct patient care and adjusts nursing processes as needed.
• Identifies problems, revises care plan, and evaluates outcomes.
• Advises and consults with family and other personnel as appropriate.
• Maintains ongoing patient assessments and teaching.
• Completes documentation including visit assessments and orders promptly.
• Communicates with physicians to update the plan of care.
