Job summary

Oak Street Health is seeking a Registered Nurse, Case Manager to drive care coordination in a field-based, on-site role across its Bethlehem, Allentown and Harrisburg clinics. You will partner with PCPs and interdisciplinary teams to manage complex patient panels and prevent avoidable readmissions.

The role emphasizes structured clinical assessments, medication reconciliation, and timely transitions of care, with opportunities for CCM certification and professional growth.

Qualifications

• Current RN license in assigned state is required.
• Bachelor degree in nursing preferred.
• Certified Case Manager (CCM) required, or willingness to obtain within 12 months of hire.
• 2+ years experience in transitional nursing, emergency room nursing, care coordination, discharge planning, or home health is strongly preferred.
• Demonstrated skill in motivational interviewing, patient activation, time management, and navigating community and social resources.

Responsibilities

• Manage an assigned caseload of complex patients in a value-based care environment.
• Accountable for panel metric performance in admission and readmission prevention, and transitions of care.
• Own overall care coordination for assigned patients and serve as primary point of contact for the care team.
• Lead transitions of care episodes and ensure timely completion of assessments and medication reconciliation.
• Collaborate with PCPs, families, social workers, and other care team members to evaluate needs and plans.
• Document visits in the electronic health record according to internal standards.
