Job summary

Oak Street Health is seeking an RN, Case Manager for a field-based, on-site role in the Walnut Hills area. The position focuses on care management, coordination across the care team, and transitions of care for complex patients in a value-based model.

Required are a current RN license and 6–8 years of nursing experience, with CCM certification or willingness to obtain within 12 months. Travel within communities served is expected.

Qualifications

• Current RN license in assigned state is required.
• CCM certification required or willingness to obtain within 12 months of hire unless candidate has 2–3 years of relevant care/case management experience.
• Minimum 6–8 years nursing experience.
• 2+ years experience in transitional nursing, emergency room nursing, care coordination, discharge planning, or home health.
• Strong motivational interviewing skills and ability to navigate social resources.

Responsibilities

• Manage an assigned caseload of complex patients to reduce admissions and resource use.
• Own care coordination and serve as primary contact across the care team.
• Oversee transitions of care and complete structured clinical assessments post-discharge.
• Collaborate with PCP, family/caregivers, Social Worker, Behavioral Health Specialists, and others.
• Document visits in EHR according to internal standards.
• Lead in-person interdisciplinary care planning meetings.
