Job summary

Oak Street Health is hiring an RN, Case Manager to lead care management for complex patients with a focus on reducing admissions and enabling seamless transitions of care. This field-based role collaborates with PCPs, social workers, and other care team members to ensure high-quality, patient-centered care.

The ideal candidate has an RN license, 6–8 years of nursing experience, and CCM certification or plans to obtain it within 12 months. Travel across OSH communities is required.

Qualifications

• Current RN license in assigned state; BSN preferred.
• 6–8 years nursing experience.
• Certified Case Manager (CCM) required, or willing to obtain within 12 months.
• 2+ years in transitional nursing, care coordination, discharge planning, or home health.
• Strong motivational interviewing and patient activation skills.
• Access to reliable transportation and ability to travel within OSH communities.
• US work authorization.

Responsibilities

• Lead care management activities with PCP collaboration.
• Drive care coordination and reduce avoidable admissions.
• Own care coordination across the care team for assigned patients.
• Manage transitions of care episodes and post-discharge needs.
• Document key assessments and maintain timely communication.
• Lead in-person interdisciplinary care planning meetings.
