Job summary

Methodist Le Bonheur Healthcare is seeking a Case Manager I to coordinate patient-centered care from pre-admission through post-discharge, ensuring safe transitions and optimal outcomes in the Memphis area.

You will work with physicians, nurses, social workers, and payors to plan care, identify barriers, and educate patients and families. RN licensure and 1–3 years of clinical experience are preferred for this role.

Qualifications

• Associates Degree in Nursing required; Bachelor's degree preferred.
• 1–3 years clinical healthcare experience (acute or hospital).
• Strong collaboration with interdisciplinary team and effective communication with patients and families.

Responsibilities

• Coordinates delivery of clinical services to achieve patient outcomes, quality indicators, and cost-effectiveness goals.
• Conducts initial assessments to identify readmission risks and barriers to transition planning.
• Makes and evaluates transition plans for patients within caseload and communicates with stakeholders.
