Job summary

Methodist Le Bonheur Healthcare in Memphis is seeking a Case Manager to coordinate care for an assigned group of patients and promote collaboration among patients, families, physicians, and the health care management team.

You will assess, implement, coordinate, monitor and evaluate services to promote quality and cost-effective outcomes, while managing authorizations and length of stay. RN experience required with 1–3 years in clinical healthcare; TN/AR/MS RN licenses preferred.

Qualifications

• Bachelor's Degree Nursing preferred.
• Associates Degree Nursing required.

Responsibilities

• Coordinate and participate as part of the Case Management team to support quality patient care.
• Identify patients that meet criteria for case management services and communicate with stakeholders.
• Plan to meet patient needs, manage authorizations and length of stay, and promote efficient utilization of resources.
• Document and track assigned cases and collaborate with physicians and health professionals.
