Job summary

BayCare Health System is seeking a Hospital Care Coordinator-RN Case Management for South Florida Baptist Hospital in Plant City, FL. This PRN, day-shift role involves linkage, planning, and advocacy to maximize patient functioning, with a focus on resource optimization and coordinated discharge planning.

Requirements include RN licensure and 2 years of RN acute care experience. ACM/ CCM is preferred; an Associates degree in nursing is required with a Bachelor's preferred.

Responsibilities

• Provides linkage, monitoring, planning and advocacy for the population to achieve maximum functioning.

• Maintains and manages assigned caseload based on needs and preferences of the served individual.
• Coordinates discharge planning through assessment, referrals and interdisciplinary planning.
• Adapts to changing priorities in a fast-paced environment.
