Qualifications

• Licensed RN in Florida; BSN preferred.
• Minimum 2 years of recent RN clinical experience.
• Prior case management experience preferred; BSN/MSN within 3 years if hired after 1/2018.
• Specialty certification within 2 years of employment for Case Managers hired after 1/2018.

Responsibilities

• Assess, plan, implement and monitor a comprehensive care plan for a defined patient group.
• Collaborate with physicians, social workers, PT/OT staff, pharmacists, respiratory, dietary and nursing teams.
• Focus on progression of care and safe, efficient transition/discharge from hospital.
