UF Health seeks an RN Case Manager to assess, plan, implement, and monitor a comprehensive care plan for a patient group, collaborating with physicians, social workers, PT/OT, pharmacists and nursing staff.

The role emphasizes progression of care during hospitalization, resource utilization, and a safe, efficient transition and discharge process for patients. A Florida RN license is required, with BSN preferred and at least two years of clinical RN experience.

Formación

• Licensed RN in Florida, BSN preferred.
• Minimum 2 years recent RN experience in a clinical capacity.

Responsabilidades

• Assess, plan, implement and monitor a comprehensive plan of care for designated patients in collaboration with the medical team.
• Coordinate with physicians, social workers, PT/OT staff, pharmacists, respiratory, dietary and nursing to ensure safe and efficient care.
• Focus on patient progression of care throughout hospitalization and assist with appropriate utilization of resources and a safe discharge.
