Job summary

UnitedHealth Group is seeking a Nurse Case Manager in Florida to guide patients through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care.

Under the supervision of an RN or MD, you will help ensure patients receive the appropriate level of care; provide education, coaching, and treatment-decision support; and collaborate with a Care Advocate and Social Worker.

Qualifications

• Current RN license valid in assigned state.
• Valid driver’s license with reliable transportation.
• Current CPR certification.
• At least 1 year of clinical RN experience.

Responsibilities

• Assess patient needs and develop plan of care with the care team.
• Coordinate care and discharge/transition planning.
• Supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Communicate patient progress to the clinical manager and care team.
• Provide staff education.
• Participate in on-call duties.
• Contribute to clinical development and continuing education.
