Job summary

Northeast Georgia Health System in Gainesville, GA seeks an experienced Registered Nurse to provide comprehensive assessment, planning, and care coordination across patient populations, from neonate to geriatric. This role collaborates with physicians, patient/family, nursing staff and the interdisciplinary team to ensure quality, cost-effective outcomes.

The RN will manage length of stay, discharge planning, regulatory compliance, and participate in rounds.

Qualifications

• Licensed to practice as an RN in Georgia.
• Associate degree; graduate of an accredited school of nursing.
• 3–5 years of experience in direct patient care and/or case management; CCM/ACM preferred.

Responsibilities

• Monitor patients on assigned units and manage length of stay.
• Coordinate discharge planning and ensure post-acute needs are arranged.
• Communicate discharge plan with physicians, patients and care teams.
• Provide hand-off communications during transitions between units.
• Maintain knowledge of reimbursement methodologies and payer guidelines.
• Collaborate with Utilization Review Nurse to ensure appropriate status and level of care.
