UF Health is seeking an RN Case Manager to assess, plan, implement and monitor care for a designated patient group within our hospital teams. You will collaborate with physicians, social workers, PT/OT, pharmacists, and dietary staff to coordinate resources and ensure a smooth transition from acute care.

• Active Florida RN license required.
• BSN preferred.
• Minimum 2 years of recent RN experience in a clinical setting.

Responsabilidades

• Assess, plan, implement and monitor a comprehensive plan of care for a group of patients.
• Collaborate with physicians, social workers, PT/OT, pharmacists, respiratory, dietary and nursing teams.
• Focus on patient progression and appropriate utilization of resources and services.
• Support safe, effective and efficient discharge planning and transitions from the hospital.
