Job summary

Moments Hospice is seeking a dedicated RN Case Manager to improve comfort during patients' final days, weeks and months. You will collaborate closely with the owners, fostering growth and clinical advancement within a supportive, entrepreneurial environment.

As an RN, you will assess patients, plan and provide nursing care, educate families, and coordinate multidisciplinary care. A state RN license and reliable vehicle are required.

Qualifications

• Licensed registered nurse in the applicable state.
• Proficiency with EMR software.
• Excellent planning, organizing and communication skills.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families and healthcare team.
• Coordinate care for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
