Qualifications

• Licensed RN in state of practice.
• Experience with EMR systems.
• Excellent planning, organizing, and communication skills.
• Valid Driver's License and reliable vehicle.

Responsibilities

• Perform initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open communication with patients, families, and health team.
• Coordinate care for caseload across interdisciplinary team meetings.
