Moments Hospice is seeking an RN Case Manager to deliver compassionate in-home or on-site care, coordinating patient plans and education. You will work closely with leadership to advance clinical growth and ensure high standards of care within the team.

The role emphasizes autonomous problem solving, strong communication, and collaboration with families and healthcare professionals to create comfortable, well-managed end-of-life care plans.

Qualifications

• RN license in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
