Job summary

Harbor Healthcare System in Clear Lake, TX seeks a part-time Registered Nurse Case Manager to join the outpatient care team. The RN will admit and coordinate appropriate care for each patient and family as part of an interdisciplinary team. Qualifications

• Texas RN license required
• 1+ years RN experience preferred
• Hospice or Home Health experience preferred

• Admissions, assessments, discharges and pronouncements as part of coordinating patient care.
• Coordinate care for each patient and family within an interdisciplinary team.
