Job summary

Harbor Healthcare System is seeking a qualified RN Case Manager for a full-time, outpatient role in the Liberty area. The RN will admit and coordinate care for each patient and family, performing admissions, assessments, discharges and pronouncements as part of an interdisciplinary care team.

Texas-licensed RNs with independent practice and strong communication skills are preferred, with travel required and a clean background check. Hospice or Home Health experience is a plus.

Qualifications

• Licensed RN in the State of Texas is required.
• 1+ years' experience as a RN is preferred.
• Experience with Hospice or Home Health is preferred.
• Effective written and verbal communication.
• Ability to travel, must have reliable transportation.
• Clean background, drug screen, and driving record.

Job Description:

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.
