Job summary

Harbor Healthcare System is seeking an experienced RN Case Manager for an outpatient role in Texas. The RN-Case Manager coordinates admissions, assessments, discharges and pronouncements, and works with the interdisciplinary care team to ensure appropriate patient and family care.

Requirements include a Texas RN license, 1+ years of RN experience (preferred), and travel ability with reliable transportation. A clean background, drug screen, and driving record are required.

Qualifications

This is an out-patient position, that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Manager main job duties include and are not limited to; admissions, assessments, discharges and pronouncements.

Qualifications
