Job summary

Harbor Hospice of Dallas is seeking a qualified Registered Nurse (RN) Case Manager for an outpatient role in the Dallas area. The RN will coordinate admissions, assessments, discharges and pronouncements as part of an interdisciplinary team.

Requirements include Texas RN license and travel ability; hospice/home health experience is preferred. Strong communication skills and reliable transportation are essential for this role.

Qualifications

• Texas RN license required.
• 1+ years RN experience preferred.
• Hospice or home health experience preferred.
• Excellent written and verbal communication.
• Must have reliable transportation for travel.
• Clean background, drug screen, and driving record.

Responsibilities

• Coordinate admissions, assessments, discharges and pronouncements for patients and families.
• Participate in interdisciplinary care planning and ensure smooth care transitions.
• Coordinate and support the care team to implement patient care plans.
• Provide patient and family education and support throughout the care episode.

Job Description:

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.

Qualifications:

• Licensed RN in the State of Texas - required
• 1+years'experience as a RN - preferred
• Experience with Hospice or Home Health- preferred
• Effective written and verbal communication
• Ability to travel, must havereliable transportation
• Clean background,drug screen, and driving record
