Job summary

Harbor Healthcare System in Beaumont, TX is seeking an experienced RN-Case Manager for outpatient care. The role involves coordinating admissions, assessments, discharges, and pronouncements as part of the interdisciplinary team.

Must hold Texas RN license, have strong communication, be able to travel with reliable transportation, and pass background checks. Hospice or home health exposure preferred; direct deposit pay, 401(k) benefits included.

This is an out-patient position, that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.

Qualifications:

• State of TexasRN license - required
• 1+ years'experience as a RN - preferred
