Job summary

Harbor Healthcare System in Texas is seeking a qualified full-time Registered Nurse (RN) Case Manager in the Lufkin area. This outpatient role requires an independent, experienced RN to coordinate patient care, admissions, assessments, discharges and pronouncements.

Responsibilities include collaborating with the interdisciplinary care team, ensuring smooth transitions, and maintaining high standards of professional communication.

Qualifications

• Licensed RN in the State of Texas - required.
• 1+ years' experience as a RN - preferred.
• Experience with Hospice or Home Health - preferred.
• Effective written and verbal communication.
• Ability to travel, must have reliable transportation.
• Clean background, drug screen, and driving record.

• Admit and coordinate care for each patient and family.
• Assessments, discharges and pronouncements in coordination with the care team.
• Coordinate with interdisciplinary care team to manage patient care and transitions.
