Job summary

Harbor Healthcare System is seeking a qualified RN Case Manager for an outpatient position in the Clear Lake area. The RN will be part of an interdisciplinary care team, handling admissions, assessments, discharges, and pronouncements, while coordinating patient care with families.

The ideal candidate will hold a Texas RN license, have strong communication skills, and be able to travel as needed. A clean background, drug screen and driving record are required, with on-site work hours Monday

We are currently seeking qualified candidates for a full time Registered Nurse (RN) Case Manager positionin the Clear Lakearea.

Job Description:

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.

Qualifications:

• Licensed RN in the State of Texas - required
• 1+years'experience as a RN - preferred
• Experience with Hospice or Home Health- preferred
• Effective written and verbal communication
• Ability to travel, must havereliable transportation
• Clean background,drug screen, and driving record
