Job summary

HarborHCS is seeking a qualified Registered Nurse Case Manager for an outpatient position in the Corpus Christi area. The RN will work with the interdisciplinary team to admit, assess, and coordinate care for patients and families, including admissions, assessments, discharges, and pronouncements.

The role requires Texas RN licensure, strong communication skills, and the ability to travel with reliable transportation; a clean background and drug screen are also required.

Qualifications

• Texas RN license required.
• Minimum 1 year RN experience preferred.
• Hospice or Home Health experience preferred.
• Strong written and verbal communication skills.
• Ability to travel with reliable transportation.
• Clean background, drug screen, and driving record.

Responsibilities

• Admit and coordinate patient care with the interdisciplinary care team.

The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family.
