Harbor Hospice of Alexandria is seeking a qualified full-time Registered Nurse Case Manager in the Alexandria area. You will be an integral part of the interdisciplinary care team, coordinating admissions, assessments, discharges and pronouncements for patients and families.

Ideal candidates hold a Texas RN license with at least 1 year of RN experience preferred, and Hospice/Home Health experience is a plus.

Competenze

• Licensed RN in the State of Texas is required according to the posting.
• 1+ years' RN experience is preferred.
• Experience with Hospice or Home Health is preferred.

Mansioni

• Coordinate admissions, assessments, discharges and pronouncements.
• Admit and coordinate appropriate care for each patient and family.

We are currently seeking qualified candidates for a full time Registered Nurse (RN) Case Manager positionin the Alexandria area.

Job Description

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.

Qualifications

• Licensed RN in the State of Texas - required
• 1+years'experience as a RN - preferred
• Experience with Hospice or Home Health- preferred
• Effective written and verbal communication
• Ability to travel, must havereliable transportation
• Clean background,drug screen, and driving record
