Job summary

Harbor Hospice of McAllen is seeking a qualified Registered Nurse (RN) Case Manager for an outpatient role in the McAllen area. The RN will be part of an interdisciplinary care team, coordinating admissions, assessments, discharges, and pronouncements to ensure seamless patient care.

Candidates must hold a Texas RN license, have strong written and verbal communication, and be able to travel with reliable transportation.

Qualifications

• Licensed RN in Texas - required
• 1+ years RN experience - preferred
• Hospice or Home Health experience - preferred
• Effective written and verbal communication
• Ability to travel with reliable transportation
• Clean background, drug screen and driving record

Job Description:

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.
