Job summary

Harbor Healthcare System is seeking a qualified RN Case Manager for a full-time role in the Victoria area. This out-patient position requires an experienced RN to coordinate admissions, assessments, discharges and pronouncements as part of the interdisciplinary care team.

Ideal candidates will hold an active Texas RN license, have 1+ year of RN experience, and preferably hospice or home health exposure. Willingness to travel and valid transportation are required.

Qualifications

• Active Texas RN license
• 1+ years RN experience (preferred)
• Hospice or Home Health experience preferred
• Effective written and verbal communication
• Ability to travel with reliable transportation
• Clean background, drug screen, and driving record

We are currently seeking qualified candidates for a full time Registered Nurse (RN) Case Manager positionin the Victoriaarea.

Job Description:

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.

Qualifications:

• Licensed RN in the State of Texas - required
• 1+years'experience as a RN - preferred
• Experience with Hospice or Home Health- preferred
• Effective written and verbal communication
• Ability to travel, must havereliable transportation
• Clean background,drug screen, and driving record
