Job summary

HarborHCS in South Austin is seeking a qualified full-time Registered Nurse (RN) Case Manager to coordinate patient care in an outpatient setting.

The RN will perform admissions, assessments, discharges, and pronouncements as part of an interdisciplinary care team; travel to patient homes may be required and a reliable vehicle is needed.

Strong communication, Texas RN license, and prior hospice or home health experience are preferred.

Qualifications

• Licensed RN in Texas (required).
• 1+ years experience as an RN preferred.
• Experience with Hospice or Home Health preferred.
• Effective written and verbal communication skills.
• Ability to travel and have reliable transportation.
• Clean background, drug screen, and driving record.

Responsibilities

Job Description:

This is an out-patient position,that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Managermain job duties include and are not limited to;admissions, assessments, discharges and pronouncements.
