Position Overview

The Hospice RN Case Manager provides coordinated, patient‑centered care to individuals and families facing life‑limiting illness. You will serve as the primary clinical guide for your patients—delivering skilled nursing care, managing symptoms, providing education, and ensuring care aligns with hospice philosophy and patient goals.

Essential Responsibilities

• Complete comprehensive patient assessments and develop individualized plans of care
• Serve as the primary clinical liaison for patients, families, physicians, and hospice team members
• Provide skilled nursing visits, symptom management, and compassionate end‑of‑life care
• Educate patients and families on disease progression, comfort measures, and available support services

Qualifications

• Current Registered Nurse (RN) license in the State of Texas, in good standing
• Hospice experience preferred(home health or case management experience welcomed)

• Bilingual (English/Spanish) a plus, reflecting the community we serve
