Pasco Southwest Home Health is seeking a dedicated Hospice RN to serve as a trusted Case Manager, delivering in-home hospice care and guiding families through decisions with compassion and dignity.

You will assess patients, develop care plans, administer medications, and coordinate with physicians and an interdisciplinary team to ensure quality, patient-centered care across home settings.

Formación

• Current RN license in the State of Colorado.
• Minimum of 2 years nursing experience; hospice or home health experience preferred.
• HCHB experience preferred, not required.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and a passion for patient-centered care.
• Ability to work independently and a passion for making a real difference.

Responsabilidades

• Complete comprehensive assessments of patients and families to determine hospice needs
• Develop and implement individualized care plans, revising as needed
• Provide skilled nursing care, administer medications, and manage symptoms
• Educate patients and families on care techniques, medications, and comfort measures
• Maintain accurate clinical documentation and communicate changes to physicians
• Participate in interdisciplinary team meetings and quality improvement initiatives
• Provide emotional support and counseling to patients and families
• Supervise hospice aides and delegate responsibilities appropriately
• Collaborate with an interdisciplinary team that shares your commitment to excellence
• Participate in on-call rotation as needed

What You’ll Do

What We’re Looking For

Salary: $ 40.00 - $52.00 per hour
