Job summary

Comfort Hospice in Nevada is seeking a dedicated Hospice RN to serve as a trusted Case Manager, delivering compassionate home-based care. You will guide families through care decisions with dignity and support during life’s delicate moments and travel throughout the Pahrump area for in-home visits.

You’ll work within an interdisciplinary team, maintain strong clinical documentation, and participate in ongoing training and quality initiatives.

Qualifications

• Current Nevada RN license.
• Minimum 2 years nursing experience; hospice/home health preferred.
• Homecare Homebase (HCHB) experience preferred, not required.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
• Collaborate with an interdisciplinary team that shares your commitment to excellence.
• Participate in holiday on-call rotation as needed.

Requirements

• Current RN license in the State of Nevada
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid driver’s license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and a passion for making a real difference
