Job summary

The Pennant Group, Inc. is seeking a Hospice RN Case Manager in Colorado to deliver home-based nursing care and guide families through care decisions within a supportive, interdisciplinary team.

You will coordinate with physicians, provide comfort and symptom management, and ensure thorough documentation while maintaining patient-centered care in patients' homes and communities. On-call rotations may be required and professional development is encouraged.

Qualifications

• Current RN license in the State of Colorado.
• Minimum of 2 years of nursing experience; hospice or home health experience preferred.
• Homecare Homebase (HCHB) experience preferred, not required.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and a passion for patient-centered care.
• Ability to work independently and a passion for making a real difference.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
• Collaborate with an interdisciplinary team that shares your commitment to excellence.
• Participate in on-call rotation as needed.

What You’ll Do

Salary: $57.00 - $59.00 per hour
