Job summary

Moments Hospice is seeking an experienced RN Case Manager to serve as the clinical lead for patients and families in a field-based capacity throughout Wisconsin. You will assess needs, coordinate care, manage symptoms, and guide families through end-of-life care with compassion.

This full-time role offers a supportive team, reasonable caseloads, and a fleet car program to support patient visits. Schedule is Monday–Friday, 8:30 AM–5:00 PM, with opportunities to shape your career while delivering

Qualifications

• Active RN license in the state you work in.
• Proficiency with EMR systems.
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver\'s license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
