Pikes Peak Hospice and Palliative Care has an immediate opening for an RN Case Manager.
STATUS: Full-time
SHIFTS: Sunday - Thursday, 8:00a - 5:00p, 40 hours
Take Care Of You Program: Scheduled 72 hours per pay period, receive 80 hours of pay.
PAY RANGE: $77,376 - $90,916 annually
SUPPLEMENTAL PAY: Based on position, schedule and/or availability: Paid mileage/Shift Diffs/Stipends
MILEAGE AND EXPENSE REIMBURSEMENT: Sixty‑two and a half cents per mile – one of the highest in the industry!
Oversees and coordinates patient and family care in accordance with hospice philosophy and delivery of care. Provides skilled nursing, including intervention, documentation, supportive care and assessments for assigned patients. The RN Case Manager is a member of the Interdisciplinary Team/Group (IDT/IDG).
